Thanks everyone for the suggestions given around here. I'm currently having this problem on Heroku with Rails 3.2.6. When RACK_ENV=development or RAILS_ENV=production everything is fine, but when RACK_ENV=production and RAILS_ENV is unset, as I prefer it to be, then I get this message. The discussion here helped me find this unique combination of circumstances.
