For the Intel HD 4000 to actually support the three independent display configuration, the following criteria need to be met: If two of the displays are DisplayPort* monitors ; If one of the displays is an Intel® Wireless Display (Intel® WiDi)

It is a Fujitsu laptop with an Intel HD Graphics 4000 and an LVDS connector. I have two monitors connected to the laptop. The laptop is Display 1 (built-in display), Left monitor is Display 2 (DVI-to-HDMI connection), Right monitor is Display 3 (VGA-to-VGA connection). However, it seems I can only extend to only a 2nd monitor.
