I played this game years back on 360, and then again a couple years ago on PC. I found this game to be drastically better with a controller... Did any recent patches add controller support or am I stuck using one of the 3rd party programs?

I did experiment using a controller with this, and it wasn't bad actually. I could not mimic some things that Sacred 2 did on the consoles, but as far as movement and combat my trick worked. Google xpadder, and get the free edition. You'll have to remap everything to the controller, but it is easy enough.
