I'm trying to do the same thing as the OP. My machine is a thinkcentre M57p (9088-A2U) with a lenovo L-IQ35 motherboard. I have a 9 pin usb header plug on the card reader, but the motherboard only has 11 pin usb header connections.

On lenovo models there are only 3 connections on a front panel, the pwr button, hdd indicator, and power indicator. I was able to trace each pair of wires to the front of the case and them match them to the connector on the mobo.
