May 06, 2014 · Dark Souls II does not recognize or take input from my controller If you are experiencing this problem, try the following: "Open up Device Manager and under Human Interface Devices, right click and disable all HID-compliant game controllers, and if you have a G13 disable the Logitch G13 Joystick. Restart the game and it should work.

Feb 19, 2017 · Epilogue: I use a third-party app for my Xbox One + Dark Souls. To be more specific I use a controller support preset for Dark Souls [www.rewasd.com]. Annnnd I heard the next version of this app will support DS3 and DS4 soon, there's no release date for now, though.
