Jun 23, 2018 · This looks helpful. I searched for the sample affidavit form from VJ and used it when i submitted my roc package few months back. I used the same sample for two friends which were both notarized, the contents were different but format is same. will this be issue? Since i am asking favour from them to complete the form, i send same sample to both.
